I understand that participating in the 2025 EIGHTY4SPORTS Football Camp involves physical activity and inherent risks of injury, illness, or property damage. These risks include, but are not limited to, sprains, fractures, head injuries, heat-related illness, and in rare cases, permanent disability or death.

I voluntarily choose to participate and assume full responsibility for any and all risks. I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless EIGHTY4SPORTS, its owners, officers, employees, agents, sponsors, and affiliates from any and all claims, liabilities, or demands of any kind, in law or equity, related to the 2025 Football Camp.

In the event of an injury or illness, I authorize EIGHTY4SPORTS and its representatives to obtain necessary medical treatment and accept responsibility for any related expenses.

I grant EIGHTY4SPORTS the right to photograph, videotape, and/or record me (or my child) during camp for promotional, commercial, and editorial use. I waive any rights to royalties or compensation.

I agree to adhere to the EIGHTY4SPORTS code of conduct. I understand failure to do so may result in dismissal from the camp without refund.

I have read this waiver and fully understand its terms. I sign voluntarily.
